Why the Scale Doesn't Tell the Whole Story on GLP-1 Medications

If you're taking a GLP-1 medication like semaglutide or tirzepatide, you've probably watched the number on the scale move — often quite quickly. It's easy to assume that number tells you everything you need to know about your progress. But weight loss isn't just about how much you lose — it's about what you're losing.
At Gippsland Dietetics, I use body composition analysis (via the InBody230) alongside GLP-1 support to help clients understand what's actually happening beneath the surface — because not all weight loss is equal, and protecting muscle matters just as much as losing fat.
What Are GLP-1 Medications, and Why Does Composition Matter?
GLP-1 receptor agonists, initially developed for type 2 diabetes, mimic a gut hormone to regulate appetite, digestion, and blood sugar. Their strong appetite-suppressing effect has made them popular for weight management. However, this can lead to unintentional under-consumption of protein. Without sufficient protein or resistance exercise, the body may break down muscle tissue for energy in a calorie deficit. Research indicates that a significant portion of weight lost with GLP-1 therapy can be from lean mass if nutrition and strength training are neglected. This is crucial because muscle supports metabolism, blood sugar regulation, and physical resilience.
Why a Bathroom Scale Can't Show You This
A standard scale gives you one number. It can't tell you whether the 2kg you lost this month was fat, muscle, or water — and that distinction has real consequences.
This is where body composition analysis comes in. The InBody230 I use in clinic breaks your results down into components a scale simply can't show, including:
● Skeletal muscle mass – are you preserving the muscle you have?
● Percent body fat – a clearer picture than weight or BMI alone
● Visceral fat level – fat stored around your organs, linked to metabolic health
● Basal metabolic rate (BMR) – roughly how many calories your body needs at rest
Rather than treating weight loss as the only marker of success, this gives us something far more useful: a picture of how your body is changing, so we can adjust your plan before muscle loss becomes a problem rather than after.
How I Use This With Clients on GLP-1s
I'm not about chasing a number for the sake of it — my approach is weight-neutral, meaning the goal isn't simply “lose as much as possible,” but supporting your health and body composition in a sustainable way. For clients using GLP-1 medications, that usually looks like:
Baseline scan first. Before we set any goals, an InBody230 scan gives us a starting point — muscle, fat, and metabolic markers — so we're not just guessing.
Protein as a priority, not an afterthought. Because appetite is suppressed, protein intake often drops without people noticing. We work out realistic ways to get enough protein across the day, even when hunger cues are quiet.
Movement that protects muscle. Resistance-based movement — appropriate to your ability and preferences — helps signal to the body that muscle is worth keeping, even in a calorie deficit.
Regular check-ins, not just at the start. Repeating InBody scans every few weeks lets us see trends over time — is muscle holding steady while fat mass drops? If muscle starts declining faster than expected, we can adjust protein targets, movement, or flag it for discussion with your prescriber, rather than finding out months later.
The Bigger Picture
This isn't about fear-mongering GLP-1 medications; they can be genuinely helpful. It's about ensuring bodily changes support long-term health, not just weight loss. Muscle loss can lead to a slower metabolism, reduced strength, and increased vulnerability to illness. Early detection with real data allows for timely adjustments. If you're using GLP-1 medications or considering them, understanding your body composition through an InBody230 scan and a personalized nutrition plan ensures your progress benefits your health.
